A record can contain a great deal of information and still be difficult to use. Readability matters because the person reviewing it may not be the person who carried out the work.

## Give the reader a route

Start with a clear identity: which batch, which product, and which version of the relevant instructions? Then use a consistent sequence for the work and the associated observations. An explicit reference is easier to follow than “same as last time.”

## Make changes visible

When a record contains a deviation, observation, or follow-up question, place it where a reviewer can find it. A readable record helps people understand what happened; a quality system determines how it should be reviewed and acted on.

## Follow the batch through the references

FDA's [cosmetics GMP checklist](https://www.fda.gov/cosmetics/cosmetics-guidance-documents/good-manufacturing-practice-gmp-guidelinesinspection-checklist-cosmetics) asks about records of material kinds, lots and quantities, processing and handling, sampling, adjustments, and batch coding. These categories give a reader concrete points to follow through a record.

In a readability review, pick one material reference and follow it from the input record to the batch instructions and observations. If the connection depends on memory, identify the missing reference. Treat this as a way to find communication gaps; the facility's quality procedures determine what records and approvals are required.

## A record is part of a larger system

FDA's cosmetic GMP checklist includes production and control records among the areas an establishment should consider. It connects records with materials, operations, and controls. **An editorial lens**

Read the record as if you had just joined the team. Which references are clear? Which sentences depend on knowledge that only the original writer has?

The goal here is a clearer conversation about documentation. This short article does not define a compliant record system or replace the procedures that govern an actual facility.
